What we collect through this website, how we use it, who we share it with, and the choices you have. Protected health information is covered separately by our HIPAA Notice.

Scope of this policy
This Privacy Policy explains how Liberty Compounding Pharmacy ("Liberty," "we," "us") handles information collected through this website.
This policy covers the website only. Protected health information that we create or receive as a pharmacy — your prescriptions, medication history and related records — is governed by the Health Insurance Portability and Accountability Act and is described in our HIPAA Notice of Privacy Practices. Where the two documents overlap, the HIPAA Notice controls for protected health information.
This policy does not apply to third-party websites we link to, or to information you give us in person, by telephone or by fax.
Information we collect
Information you give us
When you submit the contact form we collect the name, email address, telephone number, the role you select (patient or provider), the subject, and the message you write. You choose what to put in the message field.
Please do not include health information, prescriptions, diagnoses, insurance identifiers, Social Security numbers or payment card details in a website form. The form is not a secure transmission channel. Call the pharmacy at (888) 448-0549 to discuss anything clinical.
Information collected automatically
Our hosting provider records standard technical data when you load a page, including IP address, browser and device type, referring page, and the pages and times of your visit. This is used to serve the site, diagnose errors, and protect against abuse.
We do not knowingly collect information from children under 13. If you believe a child has provided information through the site, contact us and we will delete it.
How we use information
We use website information to:
- respond to your inquiry and follow up about the services you asked about;
- operate, maintain, secure and improve the site;
- detect, investigate and prevent fraud, abuse and security incidents;
- keep records and comply with legal, regulatory and pharmacy-licensing obligations;
- enforce our Terms of Use and protect our legal rights.
We do not sell your personal information, and we do not share it with third parties for their own advertising or cross-context behavioral advertising.
When we disclose information
We disclose website information only:
- to service providers who host the site, deliver email or provide security and analytics on our behalf, and who are permitted to use it only to perform those services for us;
- when required by law, subpoena, court order, or a lawful request by a regulator or law enforcement agency;
- to establish, exercise or defend legal claims, or to protect the rights, property or safety of Liberty, our patients, or the public;
- in connection with a merger, acquisition, financing or sale of assets, subject to confidentiality protections and this policy;
- with your direction or consent.
How we protect information
We use administrative, technical and physical safeguards appropriate to the sensitivity of the information we hold, including encrypted transport for site traffic and access controls that limit information to staff who need it.
No website or transmission method is completely secure. While we work to protect your information, we cannot guarantee absolute security, and information you send over the internet is transmitted at your own risk. Tell us promptly if you believe an interaction with us is no longer secure.
How long we keep information
We keep website inquiry information for as long as needed to respond to and document your request, and afterward only as long as required to meet our legal, regulatory, accounting and recordkeeping obligations or to resolve disputes. Pharmacy records are retained for the period Florida pharmacy law and HIPAA require, which is generally longer. When information is no longer needed we delete it or de-identify it.
Your choices and rights
You can decline to submit information through the site and contact us by phone instead. You may ask us to correct or delete the contact details you sent through a website form, and we will honor that request unless we are required or permitted by law to keep the information.
Depending on where you live, you may have additional rights over personal information — such as the right to confirm whether we process it, to access or obtain a copy, to correct inaccuracies, to request deletion, and to opt out of sale or targeted advertising. Florida residents may have rights under the Florida Digital Bill of Rights, and residents of other states may have comparable rights under their own laws.
To make a request, use the contact details below. We will verify your identity before acting and will not discriminate against you for exercising a privacy right. If we deny a request, you may ask us to reconsider. Rights over protected health information are handled under our HIPAA Notice, not this policy.
Changes to this policy
We may update this policy to reflect changes in our practices or the law. We will post the revised policy here with a new effective date, and material changes will be made prospectively. Your continued use of the site after an update takes effect indicates acceptance of the revised policy.
